In the unfortunate event of death due to accidents or natural disasters, our Accidental Death coverage helps ease financial burden by providing cash benefits directly to the insured’s beneficiaries.
Kung ang insured ay biglang pumanaw dahil sa aksidente o kalamidad, mayroon kaming Accidental Death Coverage na handang magbigay ng suportang pampinansyal sa pamilya ng insured.


PERMANENT DISABLEMENT (Panghabang-Buhay na Kapansanan)
In the event of loss of limb/s or disablement due to accidents or natural disasters, our Permanent Disablement & Dismemberment Coverage provides financial support directly to the insured to aid in recovery.
Sa pagtamo ng kapansanan na dulot ng aksidente o sakuna, may Permanent Disablement & Dismemberment coverage kami na makakatulong sa insured upang maka-recover.
MEDICAL REIMBURSEMENT (Bayaring Medikal)
If the insured sustains accidental injuries not included under our Permanent Disablement & Dismemberment coverage, our Medical Reimbursement benefit can help ease their hospital and medical expenses.

SPECIAL COVERAGES
UNPROVOKED MURDER AND/OR ASSAULT (Pagkakasangkot sa Krimen)
JuanLife covers injuries or death due to unprovoked murder or assault, as long as it can be proven that the insured did nothing to cause or provoke the crime.
Saklaw ng JuanLife ang mga injuries o pagpanaw na dulot ng pagkakasangkot sa krimen gaya ng murder at assault, basta’t mapatunayang walang ginawa ang insured na dahilan para siya ay saktan.


BURIAL ASSISTANCE (Tulong sa Pagpapalibing)
In the event of death due to an accident or unprovoked murder/assault, JuanLife helps reduce burial costs by providing financial aid directly to the insured’s beneficiaries.

Terms and Conditions
- Insured shall be the registered applicant for JuanLife Personal Accident Insurance.
- Insured should be Filipino citizens ages 18 to 60 years old; renewable up to 65 years old.
- Each Insured can avail of multiple JuanLife Personal Accident Insurance policies amounting to a maximum total combined coverage of P1,000,000 for themselves every year.
- The JuanLife Personal Accident Insurance is valid for one (1) year from date of activation.
- The JuanLife insurance shall only be valid once activated via SMS, Facebook Messenger or via the JuanLife website.
- For accidental death, the beneficiaries shall receive the maximum coverage of this policy. Types of accidental deaths are, self-inflicted accidents, vehicular accidents, accidental death due to fire.
- For injuries or permanent disablement caused by accidents, the insured shall receive 50% up to 100% coverage depending on the extent of injury. Please refer to the Inclusions Table for details.
- The Medical Reimbursement benefit covers medical expenses with supporting receipts related to accidents covered under the JuanLife Personal Accident Policy. It also covers medical expenses incurred from animal bites. This benefit can only be claimed once during the year of coverage.
- Financial Assistance coverage can be claimed if the Insured experiences temporary disablement due to accident. The maximum coverage is Php 2,500 for a 15-day disablement period. Actual claim shall be prorated per actual number of days incurred with supporting doctor’s certificate. The Financial Assistance coverage is an additional benefit to the Medical Reimbursement benefit.
- The Insured shall also receive burial assistance for accidental death in addition to the accidental death coverage.
- The Insured is covered for disability or loss of life due to unprovoked murder and/or assault (UMA) arising from robbery, hold-up, assault, and other unlawful offenses. Unprovoked means that the insured should not have done any action or issued words to encourage any harm done to themselves by the assailant. UMA is valid only if both parties have no established relationship. Please refer to the Inclusions Table for benefit details.
- The benefit of those engaged in hazardous occupation is reduced to 10% of each coverage amount should the accident happen during the time or performance of their job or duty as such.
- Accidents that happened in other countries are covered.
- In cases of multiple claims, the maximum amount shall be the coverage for accidental death, plus the burial assistance benefit. An example of multiple claims filing is when an Insured makes an initial claim for Medical Reimbursement and within the year, makes a Permanent Disablement or Accidental Death Claim. In such cases, the initial claimed amount for Medical Reimbursement shall be deducted from the total maximum coverage of the policy.
- LEGAL names of the insured and their beneficiaries, as well as other data submitted MUST be accurate for the insurance to be valid.
- Beneficiaries must be the LEGAL heirs of the insured.
- For insured who are married, the beneficiaries shall be the legal spouse or the children.
- For insured who are single, the beneficiaries shall be the parents or siblings.
- For insured who are single or common-law partnerships, the legal beneficiaries are the children.
- This insurance shall be considered null and void if the accident or the unprovoked murder and/or assault occurred while the Insured is participating in any criminal act or related offense.
- Ang insured ay ang nakarehistrong aplikante sa JuanLife Personal Accident Insurance.
- Ang insured ay dapat Filipino citizen na may edad 18 hanggang 60 taong gulang. Maaaring i-renew ng JuanLife member ang kanyang policy hanggang sa edad na 65.
- Ang insured ay pwedeng kumuha ng maraming JuanLife Insurance policies para sa kanyang sarili, basta’t ang kanyang total combined insurance coverage ay hindi lalagpas ng P1,000,000 bawat taon.
- Ang JuanLife Personal Accident Insurance ay may bisa ng isang (1) taon mula sa petsa ng pag-activate.
- Ang JuanLife Insurance mo ay may bisa lamang kapag na-activate na ito sa pamamagitan ng SMS, Facebook Messenger, o sa aming official JuanLife website.
- Ang benefit na makukuha ng beneficiaries sa accidental death or pagkamatay dahil sa aksidente ng insured ay ang maximum coverage ng policy. Ang mga uri ng accidental death ay self-inflicted accident, vehicular accident, at aksidente dulot sa apoy o sunog.
- Para sa mga injuries or permanent disablement na dulot ng aksidente, makakatanggap ang insured ng 50% up to 100% coverage, depende sa kapansanan na natamo. Tignan ang Inclusions Table para sa mga detalye.
- Para sa mga injuries na hindi saklaw ng aming Permanent Disablement coverage, mayroong Medical Reimbursement benefit na makakatulong pagaanin ang gastusin sa pagpapagamot. Importante lamang na maisubmit ang mga resibo na nagpapatunay na ang gastusin ay konektado sa aksidente na sakop ng JuanLife Personal Accident Insurance Policy. Covered din ng benefit na ito ang mga kagat mula sa hayop. Ang benefit na ito ay puwedeng i-claim ng isang beses sa taon na valid ang coverage.
- Ang saklaw ng Tulong Pinansyal ay maaaring makuha kung ang insured ay nakaranas ng pansamantalang kapansanan dahil sa aksidente at may rekomendasyon ang doktor na kailngan magpahinga. Ang maximum na coverage ay Php 2,500 para sa 15 araw na panahon ng kapansanan. Ang aktwal na claim ay prorated batay sa aktwal na bilang ng mga araw ng pahinga na may supporting doctor’s certificate. Ang saklaw ng Tulong Pinansyal ay karagdagang benefit sa ng Medical Reimbursement benefit.
- Ang insured ay makakatanggap din ng burial assistance para sa accidental death bilang karagdagan sa accidental death coverage.
- Ang insured ay covered para sa kapansanan o pagkawala ng buhay dahil sa hindi sinadyang pagpatay at/o pagsalakay (Unprovoked murder and/or assault) na nagmumula sa pagnanakaw, hold-up, pagsalakay, at iba pang mga unlawful offenses. Ang “unprovoked” ay nangangahulugan na ang insured ay hindi dapat gumawa ng anumang aksyon o naglabas ng mga salita na dahilan para siya ay saktan ng assailant. Ang UMA ay valid lamang kung parehong mga partido ay walang relasyon sa isa’t-isa. Tignan ang Inclusions Table para sa mga detalye.
- Ang benepisyo ng mga insured na may mapanganib na trabaho o hazardous occupation ay magiging 10% lamang ng bawat coverage amount kung ang aksidente ay nangyari habang ginagampanan nila ang kanilang trabaho o tungkulin.
- Sakop ng JuanLife Insurance policy ang mga aksidente na nangyari sa ibang bansa.
- Maaring mag-claim ang insured ng benepisyo ilang beses sa isang taon (multiple claims filing) subalit ang maximum na benepisyong maaari niyang makuha ay ang kanyang accidental death coverage at burial assistance benefit. Halimbawa ng multiple claims ay kapag ang insured ay nakakuha ng initial claim para sa Medical Reimbursement, at sa loob ng taon ay makakapag-claim ulit para sa Permanent Disablement o Accidental Death. In such cases, ang initial na amount na na-claim para sa Medical Reimbursement ay dapat ibawas sa kabuuang maximum coverage.
- Kailangang gamitin ang LEGAL na pangalan ng insured, pati ng mga beneficiaries, upang maging valid ang insurance.
- Ang mga pwedeng maging beneficiary ng insured ay ang kanyang mga LEGAL na tagapagmana lamang.
- Para sa insured na kasal, ang mga maaaring beneficiaries ay ang LEGAL na asawa o mga anak.
- Para sa insured na single, ang mga maaaring beneficiaries ay ang mga magulang o kapatid.
- Para sa mga insured na may kinasakama ngunit hindi kasal, ang mga legal beneficiaries ay ang mga anak.
- Walang bisa ang insurance kung ang dahilan ng pagka-aksidente ng insured ay ang kanyang pagkakasangkot sa krimen.

How to File a Claim?
- Agile Insurance Agency and Business Technologies shall be facilitating all Claims.
- The insured should notify and file the claims with Agile within 30 days after the date of the accident. Claims filed beyond the 1-month filing period will not be processed.

- The claims benefit shall be released based on the following schedule, on the condition that ALL required and valid documents have been submitted by the claimants:

- Accidental Death – within twenty (20) working days
- Unprovoked Murder and/or Assault – within twenty (20) working days
- In cases of claims other than Death, benefit shall be payable to the Insured.
- This policy shall become null and void if the Insured or his beneficiary/ies shall make any claim through fraudulent means or devices to obtain any benefit of this policy.
